	Fill out all areas of Gallery Mall Join Form completely.  This is the information that we will need to stay in contact with you.  This information will not be published within your Gallery.
	Enter the quantities of each of the items that you are requesting and add up your totals for the amount that is due with each application.
	Enter the number of photographs that you will be sending for us to scan and the corresponding amount which is $1.00 per photo.  10 photos = $10.00
	You may not want your personal information included within our website.  Some crafters may wish to remain anonymous.  Therefore, when we ask if you would like a Crafter Home Page, please write in "NO" on the form.

	When including information on payment with a credit card, all information is required, including your expiration date and signature.

Crafter "Home Page" Information Form

	Please include only the information that you want included on your home page, your Gallery of Rooms or your custom printable order form.
	If you include a telephone number, please indicate the time period to which customers may contact you (don't forget the time zone).
	Include the URL of your own website - where you want us to direct your visitors.
	Include the Theme Style of your Gallery Mall Pages.  This is the background, buttons, graphics and overall style of your personal website.  You may choose the default style which is Handmade Treasures theme.  Click here to view all of the available themes.
	Include general information about all or most of your items.  Consider this the lead in description.  Make your viewers want to see more.
	Turnaround:  Show the amount of time that it takes from the time you receive the order to the time that the customer will receive it.
	Terms and shipping:  Include what methods of payment you accept, including credit cards.  Most of us expect payment prior to shipping, but maybe you have special payment arrangements.  Include shipping charges or let us know if it's included in the price of the item.
	Please choose your primary style and theme for help in developing your Home Page.
	The Description and Keywords that you include will be tagged on your pages.  Use words that a customer would use for searching the web to find your items.  Be specific:  for example;  using three words; wedding rings, engagement rings, rings.
	The rest of the form is pretty obvious, however, when including kudos or testimonials, please remember that names cannot be published without the author's permission.  This is big no no.

Item Submission Form

	Furnish us with quality photographs, not Polaroid's, so that we may provide the best quality digital images in your Gallery
	Furnish us with decent quality digital pictures of your items, we can optimize them for you, however poor quality pictures may not come out well.  You may email them if you are faxing us or include them with your mailing of our forms.
	Image #: Name the files with your item number using letters and/or numbers only, to clearly identify all pictures and descriptions.  For example if your "shelf with Pegs" is your item #001, than name your pictures the same.  Keep in mind that your image number can be no longer than 12 numbers and or letters.  Do not include special characters.
	Website retrieval of images:  If you direct us to your website for your pictures, remember that we do not know what it looks like.  You must advise us of the name of the picture when your mouse points to it.
	Title:  The Title is the name of the item, such as "Shelf with Pegs" or "hand painted shelf".  Keep it short.
	Description:  Include as much information for each item as possible.   The description should include:  colors, sizes or dimensions, etc.  If your items are custom made to order - please let us know how long it will take you to deliver the item to the customer.  The more information you provide about the product, the easier it will be for the consumer to picture it in their hands. There is no limit to amount of text used, actually the more text the better!  This will allow the viewers to read while pictures are being loaded.  However, don't go overboard.
	Choose the category you want used for the placement of each piece if necessary.  Links will be provided from our Website Catalog to these Items on your pages.  Available categories at this time are:         Baskets      Decorative Jars      Candles & Accessories      Picture Frames      Birdhouses      Gift Ideas      Mirrors & Shelves      Seasonal & Holiday      Garden      Country Decor      Kitchen & Dining      Lanterns      Home Accents      Lamps & Small Furniture

	Additional categories may be added as necessary.  If you feel that your items do not fit into the above list of categories, please first choose a category from the list above and then list an alternate, naming the requested category yourself.  If the category is added your items may be moved to better suit your merchandise.
	If your items are large, bulky or heavy - additional shipping charges may be necessary.  Please consult your local UPS Shipper for more information.  If we are accepting orders and processing your payments, additional shipping charges must be listed or you will be personally liable for the extra charges.  Please do not include it in the price of the item.  List additional shipping charges separately.
	Please remember to let me know if an item on your pages is no longer available.  We will either need to replace it.

Item Submission Form - Page 2

	You may print as many copies of this form as needed to submit the amount of items that you are requesting be added to your booth(s).  Page 1 includes three items, Page 2 includes 5 items that's 8 items for the first two pages.  If you are submitting the traditional 12 items, than you will need two copies of page 2 and one copy of page 1.
