Please submit each change separately.  If you are replacing an item, list the information for the new item and include the item number of the product you want replaced.  Include as much information as possible and fill out form completely.  Refer to the guidelines for help.

Click on the button below to submit your request.  Use this form for each change that is requested.  When you have submitted all of your information, including the individual changes, we will review it and send you an invoice via email.  You may pay with Paypal through a link within the email or send us a check via snail mail.  Your changes will not take effect until payment is received.   You may proceed with your next change after receiving your confirmation page.
